Title: Effect of Hyaluronan (HA) and Metronidazole (MTZ) gels on human beta defensins in treatment of chronic periodontitis

Abstract
Introduction: Chronic periodontitis is an inflammatory condition of the tooth supporting structure, which prevail an immune response and results in loss of supporting tissues of the teeth. Human beta defensins-2 (hBD-2) are small cationic antimicrobial peptides which usually expressed in the presence of infection or inflammation. It contribute to neutrophil recruitment and links innate and adaptive immune responses by activating local dendritic cells.
Material and method: This randomized, double blinded clinical trial was done on 45 subjects satisfying the inclusion criteria. All the patients were assigned into three groups on the basis of the treatment given.ie Group I Patients received root surface debridement (RSD) only, Group II RSD with application of MTZ gel and Group III RSD with application of 1ml (0.2%) HA gel to the base of the periodontal pocket using insulin syringe. Gingival index (GI), Papillary bleeding index (PBI), Probing Pocket Depth (PPD), Clinical Attachment Loss (CAL) were done at baseline, 6th week and 12th week and biochemical analysis of Human beta defensins-2 (hBD-2) (ELISA)at 12th week after treatment in all groups
Results: GI were significantly reduced in Group II (0.008 and 0.008) *and in Group III (0.028 and 0.006) * when compared with Group I after 6th and 12th week respectively. PBI were also significantly reduced in Group II (<0.001and< 0.001)**and in Group III (<0.001 and <0.001)** after 6th and 12th week respectively. There was highly significant reduction in PPD in Group II after 6th weeks (<0.001) ** while only significant reduction in Group III. There was highly significant improvement in CAL in both Group II and Group III (same after 6th and 12th week ie <0.001).**Hbd-2 was significantly reduced in Group II (<0.001)** as well as in Group III (0.032)*.
Conclusion: HA and MTZ gels can be used as an adjunct to RSD. Reduction in human beta defensin-2 levels strengthens the fact it can be used as diagnostic biomarker for chronic periodontitis as well as for monitoring the progress of therapy.
